The St. Johns County Clerk of the Circuit Court and County Comptroller does not complete passport applications for renewals. We can provide a passport photo for a fee and review the completed application. For passport renewal applications, you will mail the application yourself through U.S.P.S.
The following will be required for your appointment:
Proof of Citizenship: One of below – Original AND One Photocopy

- Original or Certified Copy of Birth Certificate with raised seal and both parents’ names listed. (Birth registration or hospital certificate is not acceptable.)
- Original Naturalization Certificate.
- Adults 16+ may use previously issued, non-damaged passport book in good condition or passport card.
- Consular Birth Abroad Certification.
- Birth Certificates issued in Puerto Rico must have an issue date July 2010 or later.
Proof of Identity: One of below – Original AND One Photocopy of Front/Back

- Valid driver’s license issued not less than six months. If issued within the last six months, please provide a second form of identification. (i.e.: State-issued identification card, work or industrial identification card, school or college identification card, social security card. (Temporary license or learner’s permit is acceptable with a secondary form of identification.)
Minors 15 and Under:

- Proof of Parental Relationship Required: Original or Certified Copy of Birth Certificate with raised seal and both parents’ names listed. (Birth registration or hospital certificate is not acceptable.)
Application: (Unsigned) Please complete in black ink only (typed or handwritten).

- Obtain your DS-11 Application from Passport Forms (Travel.State.Gov).
- Please be sure that your printer does not shrink the application, (print it on ACTUAL SIZE) or widen the margins or it enlarges and cuts off the barcode.
- Only single sided applications will be accepted. Please do not print the application double sided.
- Applications with errors, corrections, cross-outs, or white out will not be accepted.
- Applicants 16-17 require one parent present with valid ID.
- Unexpired passports MUST be submitted with your application.
- If your previous unexpired passport is lost, you will also need a DS-64 (lost/stolen) form.
PASSPORT FEE: Personal check or money order ONLY payable to U.S. DEPARTMENT OF STATE. Starter checks are not accepted.

CLERK FEES: Separate Clerk fees will apply and can be paid with cash, credit card, debit card, money order, personal, or cashier’s check. If your appointment is at one of our annex locations, Ponte Vedra or Julington Creek, credit card, debit card, personal or cashier’s check will be accepted. Cash will not be accepted. (Starter checks not accepted.)

- Clerk of Court execution fee for processing and mailing is $35 PER application.
- Photos are available for $10.65 per photo.
- You may have your applications sent by EXPRESS MAIL To the processing center for $30.45. All applications will be placed in the one envelope.
