☐ Obtain Certified Marriage Certificate
After your wedding, record your marriage license with the Clerk’s Office and request a certified copy. This is the official document you’ll need to change your name.
☐ Update Social Security Card
Complete the SSA’s form SS-5, then bring the following to your local Social Security office.
- Certified marriage certificate
- Current Social Security card
- Valid Florida ID or driver license
☐ Update Your Florida Driver License or State ID
Visit your local DMV office in person, pay the fee for a replacement license, and bring the following:
- Certified marriage certificate
- New Social Security card
- Current Florida driver license or ID
☐ Update Your Passport
Fill out a Dept. of State name change form. Submit by mail along with a certified copy of your marriage license and your current passport.
- Use form DS-5504 if your passport was issued less than 1 year ago (free).
- Use form DS-82 if older than 1 year (fee applies).
- $130 for passport book
- $30 for passport card
- $160 for both
- Submit with your certified marriage license along with your current passport.
- Mail to the National Passport Processing Center.
☐ Update Your Bank and Financial Accounts
Visit your financial institution in person to present your certified marriage license along with ID.
☐ Update Your Employer and Payroll Records
Bring your new social security card and certified marriage license to HR. Be sure to update benefits, insurance and retirement accounts.
☐ Update Other Records and Accounts
- Insurance companies (auto, health, home, life)
- Voter registration (visit your local Supervisor of Elections office or online)
- Utilities, phone, internet, and service providers
- Professional licenses (check Florida DBPR at myfloridalicense.com if applicable)
- Medical providers and prescription records
- Schools, alumni associations, and other memberships
